The rules of the game have changed and our approach must follow suit.
I remember when I turned 40 and and my body was like aha! Let’s mess with a few things! My cycle got just a tad shorter, period heavier and my tried and tested strategies kind of stopped working. I felt surprised and mildly offended that all of this was happening to me, surely I was too young to deal with PERIMENOPAUSE?
The reality is for most of us, the subtle changes start happening in our late 30s and we really FEEL the effects in our 40s. The real issue is that there’s next to zero guidance on what we can do to support this change of equilibrium. This is true for all women and even more so for anyone juggling a thyroid condition. And it’s only natural to ‘blame the thyroid’ for all the weird symptoms.
But the reality is, your thyroid function, the hormone levels and autoimmunity are determined by other systems in your body, aka The Thyroid Terrain. And your fluctuating sex hormones are a key player here.
Let’s talk about the weird symptoms…
You enter your 40s and suddenly, morning anxiety hits before your eyes open. Sleep is trash. Tolerance to alcohol and exercise has gone downhill. Weight creeping up despite no changes in the diet. Slightly shorter cycles, heavier bleed and wtaf is happening with the PMS. I thought it was getting better? Brain fog and fatigue ramp up (what was i saying?). You keep adjusting your thyroid meds but the symptoms don’t budge. It kind of feels like you took 10 steps backwards. Your thyroid must be getting worse, right?
What if I told you… Your thyroid might not be the main culprit here.
It’s perimenopause changing your body’s chemistry and HOW it uses thyroid hormone.
Here’s The Lineup
Progesterone drops first (for most of us in our late 30s, early 40s, waaay before the first hot flush!)
Sleep and nervous system regulation (wired but tired anyone?).
Thyroid hormone bioavailability, leading to hypo symptoms with normal labs.
Gut microbiome (constipation, bloating and hormones recirculating).
Cortisol and blood sugar (cellular resistance to thyroid hormones, increase reverse T3).
Estrogen balance leading to functional estrogen dominance.
Your thyroid medication CAN’T fix hormone chaos. That’s why increasing your dose doesn’t help.
Then usually around your mid 40s, oestrogen starts, well… misbehaving!
It doesn’t just gradually go down, it drops and then it spikes, contributing to inflammation surges, liver & gallbladder issues, blood sugar imbalances, and exacerbated thyroid issues (you might see TSH creeping up, more conversion issues, and even antibodies going up).
Tell tale signs: crazy brain fog, dryness (everywhere), joint pain, unexplained weight gain, and pretty intense mood swings. And yes, the odd hot flush here and there!
The big realisation here is:
What worked in your 30s won’t work in your 40s.
What worked in your 40s needs further adjustments in your 50s.
It’s not YOU failing or your thyroid inexplicably getting worse despite ‘normal’ labs.
It’s your hormones changing how your body works and your strategy needs adjusting.
Here’s the thing though…
You need to take agency over this. The way you eat/live needs to be tweaked to support the hormonal change, and therefore your thyroid. It’s time to review your supplements, the way you move your body, how you rest.
These are mostly small tweaks to your existing routines but they will CHANGE EVERYTHING.
It’s also time to EDUCATE YOURSELF on HRT, smart hormone testing in peri, and natural hormone support options (I know it’s all a little confusing and no we shouldn’t be blindly throwing oestrogen at everyone 40+).
